"Unique ID for Persons with Disabilities” project is being implemented by Department of Empowerment of Persons with Disabilities, Ministry of Social Justice & Empowerment with a view of creating a National Database for PwDs, and to issue a Unique Disability Identity Card to each person with disabilities. The Objective of this project is to enable the PwDs to obtain the new UDID card / Disability Certificate to avail schemes and benefits. Persons with Disabilities having more than 1% of disability can register in UDID.
Eligibility Criteria:
(i) Persons with Disabilities who already have a Disability Certificate.
(ii) Persons with Disabilities who don’t have any Disability Certificate.
(iii) Persons with Disabilities who have applied for Disability Certificates and their Certificates are under process.
(iv) Persons with Disabilities whose applications were rejected/want to re-apply for Disability Certificates and also for re-assessment of percentage/category of Disability.

How will one know that she/he has successfully registered in UDID?
All the beneficiaries those who have registered in UDID will receive one confirmation message after successful registration with their acknowledgement number for time to time correspondence.
Where should one go for assessment?
All the beneficiaries who have registered in UDID will visit their District Hospitals as per the schedule time allotted to them.
Whether the UDID card is valid for availing Government facilities?
UDID card is valid for Persons with Disabilities for availing benefits as per Government guidelines either through Government of Odisha or Government of India.
Where will the UDID card be delivered?
UDID card will be delivered to the beneficiaries at the address mentioned by them during online registration by post.
Who monitors the UDID Process?
UDID is monitored by Government of Odisha and Government of India and this is purely a Government Project
